We are currently looking for a Triage RN to join our growing team!

Provides phone triage and support to patient/families. Schedules visits by multidisciplinary team members as needed. Performs patient care visits and admissions as needed, may include tuck in and Continuous Care visits to provide symptom management/comfort care; help with vacation coverage.

Skills / Requirements

License/Education/Qualifications Required:
Ca RN License, 1 year recent acute care experience.

Additional Qualifications Preferred:
Hospice or Home Health experience.
