We are currently looking for a RN Case Manager to join our growing team!

Responsible for performing case management of patient care in collaboration with patient/caregivers and other members of the interdisciplinary team. Coordinates the plan of care.

Skills / Requirements

License/Education/Qualifications Required: CA RN Licnese and 1 year recent acute care experience.

Additional Qualifications Preferred: Hospice or Home Health experience, hospice certification preferred.
